Moderate medical decision making is often assumed but not always supported by the documentation.

In this interactive session, participants will review real documentation scenarios and determine when a visit truly meets the requirements for 99214. Attendees will evaluate the problems addressed, the data reviewed, and the risk involved in patient management. The goal is to strengthen confidence in defensible E/M leveling and help auditors recognize when documentation truly supports moderate medical decision making.

Peer comparison through live leaderboard participation will add a special layer of interactivity to the session.

Templates can improve efficiency, but they can also introduce significant documentation risk.

In this interactive session, participants will review real documentation examples that include templated histories, copied examination findings, and standardized medical decision making language. Through discussion and polling, attendees will identify template-related red flags and determine what documentation truly supports the service reported.

This session focuses on practical strategies auditors can use to evaluate templated records with confidence and consistency.

Audits do not always end with agreement. At times, an auditor’s findings can create tension, disagreement, or even frustration from the physician involved.

In this session, participants will walk through a real-world audit scenario that resulted in significant physician pushback. Through guided discussion and live audience polling, participants will explore practical strategies for communicating audit results, maintaining professionalism, and turning a difficult conversation into an opportunity for education.

This final session brings the entire workshop together and allows participants to practice making defensible audit determinations using real-world documentation examples.

Attendees will evaluate documentation that includes E/M level determination, modifier 25 considerations, and split/shared visit scenarios. Through live polling and guided discussion, participants will determine whether the documentation supports the reported services and compare their decisions with the reasoning used by experienced auditors.
